Should a timesheet include unpaid breaks?

No. A break is not project work, so it does not belong in a project row. Record the break in the attendance record, where the day's clock times live.

What if someone worked on six projects in a day?

Add rows. A grid with six thin rows is honest; rounding five of them into a sixth is not, and it makes the biggest project look bigger.

Does a timesheet need to match the attendance record exactly?
